Abstract: This two-part video (part 1 is 8:00 minutes and part 2 is11:54 minutes) shows two transition planning conferences in which a service coordinator and preschool transition coordinator gather information from parents and discuss their child’s transition from an Individualized Family Service Program (IFSP) to an Individualized Education Plan (IEP).
